2. Please bring your insurance information to your first appointment, along with your insurance card(s).
3. Call 1-765-894-2620 to schedule the appointment. If you reach my voice mail, please leave a message.
4. Co-pays and payments must be paid cash or check on day of service. Credit card payments are not available.

On arrival (main entrance):

1. Please call your insurance company to verify your mental health benefits (ie., deductible, co-pays and number of visits). You should also verify that I am considered an in-network provider.
The main entrance faces North Lebanon Street (SR 39). There is a short flight of steps to the door. Go inside to the vestibule. There will be a button on the right wall (marked by a sign). Push the button. When someone answers, announce your name and that you will be seeing Katie Mossman. Someone will buzz you in.
